Why the gallery, of all pages
When a prospective patient evaluates a surgeon, the before-and-after gallery is the first thing she checks — before credentials, before reviews, before price signals. We've mapped that evaluation sequence in our breakdown of how plastic surgery patients actually find their surgeon: if the gallery doesn't resonate, nothing else on the site matters.
Search behavior mirrors that. “[Procedure] before and after” is one of the highest-volume query patterns in aesthetic medicine — tens of thousands of monthly searches across procedures, typed by patients deep enough into their research to want proof, not definitions. Yet most practices treat the gallery as a single monolithic page: one URL, hundreds of unlabeled photos, no structure a search engine or an AI system can read. The result is a page patients love and Google ignores.
This practice — a Los Angeles-area plastic surgery group whose identifying details we withhold, as we do in every case study we publish — had exactly that profile: strong surgical results, heavy gallery traffic from existing patients, and a fraction of the search visibility those photos deserved.
What we rebuilt
The engagement centered on rebuilding the gallery from a browsing experience into a structured, per-procedure system:
- One gallery page per procedure and body area — abdomen liposuction, waist liposuction, arm liposuction, neck liposuction, buccal fat reduction, Brazilian butt lift, tummy tuck, and more — so each page could rank for its own query family instead of one URL diluting across all of them
- Gallery UX built for comparison: consistent framing, case-by-case organization, and the browsing patterns patients actually use when evaluating whether results match their own starting point — the standard we outline in our gallery best-practices guide
- Crawlable structure and descriptive metadata, so search engines and the LLMs behind AI answers could finally read what each page documents
- Internal linking connecting each gallery to its procedure page, so a research visit has a next step toward a consultation
No link building campaign ran against these pages during the window. The movement below came from the redesign and the structure around it.
The results in detail
Rankings consolidated upward
The headline pattern is concentration at the top. Top-3 positions grew from 13 to 52, and the practice now holds 18 #1 organic positions on gallery queries where it previously held none — meaning more than a third of its top-3 rankings are outright firsts. In parallel, the long tail widened: 140 distinct ranking queries became 335, nearly 200 net-new queries entering Google's top 100.

The flagship gallery took over its query family
The abdomen liposuction gallery became the proof of concept. It now ranks in the top three for nearly every variant of its query family — “stomach lipo before and after” (#1, 1,600 searches/mo), “belly liposuction before and after” (#3, 1,900 searches/mo), “lipo abdomen before after” (#3), “tummy lipo before and after” — and holds the #1 position on several of them. That single gallery accounted for the majority of the estimated traffic gains in the window.

AI answers started citing the galleries
The newer result category: 33 of the practice's gallery rankings are now placements inside Google's AI Overviews, 19 of them earned during this window. When a patient asks Google what abdominal liposuction results look like, the AI-written answer above the organic results now cites this practice's gallery as a source. Galleries are unusually well-suited to this: AI systems need credible, structured visual documentation to point to, and almost no practice structures its galleries to be citable. The part other case studies leave out: auditing the losses
Position-tracking exports flag rankings as “lost,” and this export flagged 83 of them. A less careful case study would either hide that number or quietly subtract it. We audited every row instead, and the classification is worth showing because it changes what the number means:
- 38 of 83 “lost” rows still rank— the same keyword appears elsewhere in the same export, on a different URL or a different SERP feature. These are reshuffles, not losses: the tracking tool logs a “lost” row when a ranking moves from one surface to another.
- 40 rows were page-3-or-deeper positions (previous positions 21–99) carrying zero estimated traffic. Rankings that deep churn in and out of the top 100 constantly; losing a #85 costs nothing measurable.
- 5 rows were real losses — mostly People Also Ask features on low-volume queries — worth a combined 2 estimated visits per month.
Net effect of every flagged loss in the export: −2 estimated monthly visits, against +551 in gains on improving queries. We also verified the biggest flagged terms against live ranking data before publishing this page: the practice's top gallery rankings — including its #1 positions — are all currently live.
How we measured, and what we left out
- Data source: SEMrush keyword-level position-change exports (US database), comparing a June 15, 2026 baseline against mid-July 2026, verified against live SEMrush ranking data at publication. Third-party estimates typically run conservative versus actual search console clicks.
- Scope:only queries containing “before and after” (and close variants) are counted. Every other keyword the practice ranks for, including all local and branded queries, is excluded from every number on this page. The win/loss audit above is based on the 331-row position-change export; headline position counts reflect the full before-and-after keyword set.
- Traffic figures: SEMrush estimated monthly organic visits attributable to each ranking, netted across gains and declines rather than cherry-picked.
- The losses: disclosed and classified above rather than omitted.
Any agency showing you a gallery case study should be able to tell you the comparison window, the query scope, and what happened to the rankings that went down. If they can't, that is your answer. We keep a list of questions worth asking any agency before you sign.
What this means for your gallery
If your before-and-after gallery is one long page of photos, you are sitting on the same unrealized asset this practice was. The queries already exist — patients in your market are searching “[your procedure] before and after” thousands of times a month — and the practices winning those searches are rarely the ones with the best surgical results. They are the ones whose galleries are structured so Google and AI systems can read them.
- Search your highest-revenue procedure plus “before and after” and note who ranks — and whether an AI Overview answers first.
- Count your gallery URLs. One page covering every procedure means every procedure competes against your own site.
- Split the gallery by procedure and body area, structure each page for comparison, and connect each one to its procedure page.
- Re-measure positions and AI citations quarterly.
