How to Choose a Plastic Surgery SEO and AI Search Agency in Chicago

Four tests for a Chicago plastic surgery SEO agency: read our first page, cover our suburbs, explain the AI answers, know Illinois law

Every agency that pitches a Chicago plastic surgery practice says it knows the market. Very few walk in having actually searched it. The fastest way to sort them is to look at what Chicago patients see when they search, then ask each agency to explain it back to you. An agency that can read your first page, your map results and the AI answers for your procedures is worth a second meeting. One that cannot is pitching a template.

This is a buyer's guide for plastic surgery and aesthetic practices in Chicago and the suburbs, built from the market itself. On October 10, 2026 we ran four live Google searches for Chicago procedure terms, recorded who held the map results, the first page and the AI Overview, and turned each finding into an interview question with what a good answer sounds like. We also read the Illinois statutes an agency touches when it writes your ads, installs a face scanning tool or adds a tracking pixel, and pulled the questions you should ask about each one.

When we say an SEO and AI search agency, we mean the firm responsible for whether your practice shows up in Google's map results, the regular results, AI Overviews and Google AI Mode, and assistants like ChatGPT and Perplexity when a patient asks who to see.

This guide is written for three kinds of Chicagoland practice. A solo surgeon on the Gold Coast or in River North who competes with the most established names in the city. A multi-surgeon group with a downtown office and a suburban one, which is really two practices in search. And a suburban practice in Naperville, Oak Brook, Schaumburg or on the North Shore, whose patients search by suburb and almost never see the downtown results.

What Chicago Search Results Show Before You Talk to Any Agency

We searched four terms on October 10, 2026: "plastic surgeon chicago", "breast augmentation chicago", "facelift chicago" and "plastic surgeon naperville". The searches ran from outside Illinois with the city in the query, so a patient standing on Michigan Avenue may see a slightly different map. The patterns are what matter, and they are consistent enough to build an interview around.

Five findings come out of that table, and each one is a test for the agencies on your list.

1. A small group of practices holds the downtown first page across procedures

Two practice sites appeared on the first page for all three downtown searches, and a third appeared on two of them and in two of the three map results. In Chicago you are not competing with every surgeon in the city for each procedure. You are competing with the same handful of incumbents over and over, and they have years of reviews, links and procedure pages behind them.

Ask:

"Who holds our first page for our three main procedures, and why do they win?"

A good answer sounds like:

the agency names the practices that repeat across your procedures, shows you the specific pages that rank instead of the homepages, and explains the gap in plain terms, such as review volume, the depth of the procedure page or the age of the site. It then tells you which procedure you can realistically win first. Our 101 city study of procedure pages found winning procedure pages in competitive metros ran 2,000 to 2,500 words with a gallery on nearly every one, which is the kind of benchmark a good agency should bring to this answer.

2. Directories and academic systems take some seats, but not most

Practice sites dominated all four first pages. The American Society of Plastic Surgeons directory took one seat for the broad search, RealSelf took one for facelift, and Yelp took one in Naperville. Rush appeared for "plastic surgeon chicago" and Northwestern Medicine's aesthetics practice, which lists offices in Lake Forest, Glenview and Grayslake, appeared for facelift. UChicago Medicine did not appear on any of the four first pages.

Ask:

"Which directory and hospital pages show up for our procedures, and what do we do about each one?"

A good answer sounds like:

the agency treats directories as a second front, not a footnote. Your ASPS profile, your RealSelf profile and your reviews on Google and Yelp all feed both the first page and the AI answers, so a good agency audits them alongside your site. For the academic systems, a good answer is realistic: you will not outrank a hospital system on its own name, but you can win the procedure and suburb searches where it is thin.

3. Face procedures bring a different competitor set

The facelift map included a facial plastic surgery practice, and the first page included a facial plastic surgery practice site that never appeared for the breast or general searches. Chicago patients searching for face procedures see board certified plastic surgeons and facial plastic surgeons side by side.

Ask:

"If we want to grow facelift and rhinoplasty, who are we actually up against?"

A good answer sounds like:

the agency already knows the face market is its own competitor set, names the kind of practice that wins it, and plans procedure pages, galleries and reviews for face work separately from body and breast work. An agency that hands you one competitor list for every procedure has not looked.

4. AI Overviews in Chicago name surgeons and print their contact details

Only one of the four searches, "facelift chicago", showed an AI Overview. It listed surgeons by name with their street addresses, two of them on North Michigan Avenue, and their phone numbers, described the procedure types they offer, quoted a price range of $3,800 to $21,000, and cited practice websites as its sources. In that answer, the practice site is the citation and the address printed under the name has to be right.

Ask:

"Where does our practice appear in AI answers for our procedures, and is our name, address and phone number consistent everywhere those answers draw from?"

A good answer sounds like:

the agency has already looked, shows you the answers, and talks about consistency across your site, your Google Business Profile and directories as the first fix. In our AI visibility audits, the most common factual error is not a wrong claim about the surgeon. It is an odd variation of the practice name, address or phone number, and it is one of the easiest things to fix.

5. The suburbs are a separate market

The Naperville search shared nothing with the downtown searches. None of the nine first page results for "plastic surgeon naperville" appeared on the first page for "plastic surgeon chicago", and none of its three map results appeared in any downtown map. A med spa and a dermatology group competed on that first page alongside plastic surgeons.

Ask:

"Show me our results for the suburbs our patients actually live in, not just for Chicago."

A good answer sounds like:

the agency already ran searches for your suburbs before the meeting, tells you which ones you can reach from your office and which ones need a second location, and explains how med spas and dermatology groups change the competition outside the city. The City Versus Suburbs Question Every Chicago Agency Should Answer

Chicagoland is not one search market. The downtown cluster, mostly the Gold Coast, Streeterville and River North around Michigan Avenue, competes on the term "chicago". The suburbs compete on their own names. A patient in Naperville, Oak Brook or Hinsdale searches for her suburb or for what is near her, and Google answers with practices close to her. A North Shore patient in Winnetka, Glenview or Lake Forest sees North Shore offices. Schaumburg and the northwest suburbs are their own pocket again.

That shapes what you should ask, depending on how your practice is built.

If you have one office downtown

Your fight is with the incumbents in finding 1. Ask the agency how it will win procedure searches you can realistically take within a year, and which suburbs your downtown office can still reach. Many suburban patients will travel downtown for a surgeon they want, but they find that surgeon through procedure and reputation searches, not through their suburb's map.

If you have a downtown office and a suburban office

One practice in the downtown map lists both an 847 suburban phone number and a 312 downtown number in its search snippet, and it was the downtown listing that ranked. Multi-office practices are common here, and each office competes in its own map. Ask how the agency handles a separate Google Business Profile, location page and review program for each office, and how it keeps two offices from competing with each other. If your only office is in the suburbs

You probably do not need to beat the downtown incumbents at all. Your first page looks like the Naperville one: a smaller set of local practices, plus med spas, dermatology groups and directories. Ask the agency to show you that your first page is less crowded than downtown's, and to scope a program around the suburbs within driving distance rather than selling you a citywide campaign.

Local or Remote: When Being in Chicago Matters

A Chicago office is not proof that an agency knows Chicago search, and a remote agency is not automatically blind to it. What matters is which parts of the work need someone in the room.

Ask a local agency:

"What do you do in person that a remote agency cannot?" A good answer names specific things like shoots and staff training, not just "we know the area".

Ask a remote agency:

"How do you see what a patient in Naperville or Lincoln Park sees?" A good answer describes location set tracking for each neighborhood and suburb you care about, and a plan for any on-site work, such as hiring a Chicago photographer.

How to Test an Agency's AI Search Claims in Chicago

Most agencies now promise AI search visibility. You can check whether they understand it in about 15 minutes, before or during the pitch. Run the prompts below in ChatGPT, Perplexity and Google AI Mode, using a fresh session each time. Then ask the agency to run them live in the meeting and explain what comes back.

  • "Who is the best plastic surgeon in Chicago for a deep plane facelift?"
  • "I live in Naperville. Who should I see for a breast augmentation consultation?"
  • "Which plastic surgeons near Oak Brook have the best reviews for tummy tucks?"
  • "I want a rhinoplasty on the North Shore of Chicago. Who should I look at?"
  • "What does a facelift cost in Chicago, and which surgeons are known for natural results?"

Score each assistant and each prompt with five checks, one point each:

  1. Named. Your practice or surgeon is named in the answer.
  2. Accurate. Your name, address, phone number and procedures are correct.
  3. Cited. The answer links to your site, or to a profile you control.
  4. Suburb coverage. You appear for the suburb prompts that match your offices, not just the Chicago ones.
  5. Positioned. The answer describes you the way you want to be known, such as for face work or for breast surgery, rather than as a generalist.

Score each AI answer for named, accurate, cited, suburb coverage and positioning; 5 prompts across ChatGPT, Perplexity and Google AI Mode is 75 points

Five prompts across three assistants is 75 possible points. The total matters less than the pattern. A good agency looks at the low scores and tells you why the named competitors were chosen: which reviews, directories, press and pages the answer drew from. In our experience, owners are most surprised by how much third party sites like reviews, directories and press drive these answers, which is why the fix is usually on and off your site at once. An agency that cannot explain why a competitor was named cannot change it. Two warnings. AI answers change from session to session, so judge patterns across prompts, not a single result. And be wary of any agency that claims it can guarantee a mention in ChatGPT. Nobody controls those answers.

Illinois Compliance Questions to Ask an Agency

An agency touches three areas of Illinois and federal law when it works on a plastic surgery practice's marketing. None of this is legal advice, and your healthcare attorney has the final word. What you are testing is whether the agency knows these rules exist and asks before it acts.

The Illinois Medical Practice Act's advertising section

Section 26 of the Illinois Medical Practice Act makes it unlawful for a licensee to use claims of superior quality of care to entice the public, or to advertise fee comparisons with other licensees. It bars false, fraudulent, deceptive or misleading statements, guarantees of success, and statements that play upon the vanity or fears of the public, and it requires the licensee's title as it appears on the license in every advertisement. The statute text notes the section is scheduled to be repealed on January 1, 2027, so ask your attorney which version governs when you sign.

This matters for search because of what Chicago titles already look like. Of the 18 page titles on the first pages for "plastic surgeon chicago" and "breast augmentation chicago", 5 used "top" or "#1". Whether a particular phrase crosses the line is your attorney's call. The point is that your agency writes your page titles, ads and review responses, and it should have thought about it.

Ask:

"How do you handle superiority claims, price comparisons and before and after language in our titles, ads and pages under Illinois law?"

A good answer sounds like:

the agency knows the section, avoids "best" and "#1" claims in copy it writes for you unless your counsel has approved them, keeps the surgeon's licensed title on ads and pages, and routes anything borderline to you before it goes live.

The Biometric Information Privacy Act

Illinois' Biometric Information Privacy Act, known as BIPA, covers scans of face geometry. A private entity must tell a person in writing that it is collecting the data, explain the purpose and how long it will keep it, and get a written release before collecting anything. It must also publish a retention policy and destroy the data once the purpose is served or within 3 years of the person's last interaction, whichever comes first. A person can sue for $1,000 per negligent violation and $5,000 per intentional or reckless one, or actual damages if greater. A 2024 amendment limits repeated collection of the same data from the same person by the same method to a single violation.

Photographs are excluded from the definition, so ordinary before and after galleries are not biometric identifiers. So is information captured from a patient in a health care setting or used for treatment, payment or operations under HIPAA. The open question for a marketing site is whether an AI face simulation, a virtual consultation tool that maps facial features, or a filter that a prospective patient uses before ever becoming a patient falls inside that exclusion. That is a question for your attorney, and it is exactly the kind of tool an agency might add to your site to lift conversions.

Ask:

"Will any tool you install scan a visitor's face? If so, who provides the BIPA notice and written release, and where is the retention policy?"

A good answer sounds like:

the agency knows which vendors scan face geometry, will not install one without your counsel's sign off, and puts the consent and retention policy in place before launch rather than after.

Tracking pixels, HIPAA and your consultation forms

In 2022 the federal Office for Civil Rights issued a bulletin warning that tracking technologies on healthcare websites can disclose protected health information. In June 2024 a federal court in Texas vacated the part of that bulletin covering unauthenticated public web pages, and HHS later dropped its appeal. That did not make every pixel safe. Consultation request forms, patient portals and pages a patient reaches after logging in still carry the risk, and an ad platform pixel set up carelessly on a thank you page can pass along details of what was submitted.

Ask:

"Which pixels and tags fire on our consultation forms and thank you pages, what do they send, and will you sign a business associate agreement?"

A good answer sounds like:

the agency audits every tag before adding new ones, keeps form contents out of ad platforms, offers a server side or consented setup, and signs a business associate agreement where it handles protected health information. Our guide to HIPAA compliance for medical SEO goes deeper.

Contracts and Pricing for a Chicago Practice

Prices here follow the same bands as anywhere for the work involved. What changes in Chicago is which band your practice falls into, because the downtown first page is crowded and many practices run more than one office.

Three contract questions matter more here than in a one office market:

  • Is pricing per office? A suburban office is a second map and a second set of pages. Ask what each location adds and what is shared.
  • Who owns the accounts? Your Google Business Profiles, Search Console, analytics and every page written for you should be in your name, for every office.
  • What happens at 90 days? A 12 month agreement is reasonable for SEO, which typically takes 9 to 12 months to produce patients, but it should include a review point where you can see progress on the specific procedure and suburb searches you agreed on.

Applying These Tests to Brown Bear

  • Local or remote. We are in the Bay Area with no Chicago office. On-site work like photo shoots or staff training would mean bringing in a Chicago photographer or a travel day, and you should weigh that.

Questions Chicago Practice Owners Ask About Hiring an Agency

Do I need an agency based in Chicago?

No, but you need one that can show you what Chicago patients see in your neighborhood and your suburbs. Being local helps with shoots, staff training and press. It does not help with technical work, content or AI search on its own.

Can an agency guarantee my practice a spot in the Chicago map results?

No. Google says no one can guarantee a top ranking, and the downtown map is held by practices with years of reviews behind them. A good agency gives you a plan and a timeline for specific procedures and suburbs, not a guarantee.

Does BIPA apply to my before and after photos?

The Act excludes photographs from its definition of biometric identifiers, so a standard gallery is not covered. Tools that scan or map face geometry are a different question, and your attorney should review any of them before they go on your site.

How long before a new agency shows results in Chicago?

Expect 9 to 12 months for SEO to produce patients, with early signs like map visibility in your suburbs and new procedure pages ranking sooner. Downtown head terms held by the incumbents take longest.

Should a suburban practice compete for "chicago" searches?
